Physical Therapy Associates Rehabilitation Services is Southwest Florida's largest privately-owned outpatient rehabilitation organization serving the community since 1987. Our practice's success depends on building and maintaining mutually beneficial relationships with our referring physicians and their staffs. Communication is of utmost importance to us. After a patient is evaluated, a copy of the evaluation and plan of care will be forwarded to your office for your review. If you agree with the treatment plan, please sign it and return to our office promptly. This will prevent any delays in the patient's treatment plan. Progress summaries are also sent periodically to inform you of the patient's progress and request additional therapy if necessary. If you ever have a question or concern about your patient's treatment plan, please do not hesitate to call us.
Our Therapists Our occupational therapists have extensive experience and education in treating hand-related disorders and custom splinting as well as many neurological disorders.
Referring a patient We verify outpatient therapy benefits for each patient and obtain all necessary authorizations or pre-certifications, saving your staff time and energy. To refer a patient, simply phone the referral information or fax a signed (not stamped) referral/prescription and the patient's demographic information to any of our five locations. Our facilities
