At Physical Therapy Associates, our therapists work closely with you, your family, and your referring physician to design an individualized treatment program to maximize function and regain your independence following an injury, surgery, or illness.

Our licensed therapists have extensive training and attend continuing education courses each year to provide the most current and effective treatment techniques.
Referral Requirements:
We require a signed referral (prescription) from your treating physician. Certain insurance companies require your treatment to be pre-authorized by your primary care provider, which will be requested by our staff. To find out more about your insurance policy and its limitations on outpatient therapy services, contact your carrier directly.
How to Schedule an Appt:
Contact any of our five locations most convenient to your home or work. Initial appointments may be scheduled by the patient or referral source. Clinic hours are 7 a.m. - 6 p.m., Monday - Friday.
Your First Appointment:
At your first appointment, your therapist will obtain a thorough history of your medical condition and perform an evaluation, discuss a treatment plan, and set goals with you.
Financial Policy/Billing Information
Physical Therapy Associates accepts Medicare, HMO/PPO Insurances, Workers' Compensation, and Auto Insurance Plans. As a courtesy to our patients, we will verify your benefits before your first visit and will file charges to your insurance company. However, it is your responsibility to know your insurance policy and its limitations. You will be responsible for any co-pays, deductibles, or non-covered services set by your insurance policy.
We accept cash and checks for payment, and offer a self-pay rate for patients without outpatient therapy benefits or without insurance.
